Premium Assistance for Kids (PAK) Application

The application process for Premium Assistance for Kids (PAK) involves two steps:
Step One:
Call the Insure New Mexico! Solutions Center at 1-888-997-2583 to request a PAK
application or download here English or Spanish . Your PAK application will take up to 45
days to process. If you are financially eligible, you will receive an approval notice
with instructions to contact the Managed Care Organization (MCO) to complete the
enrollment process.
Financial eligibility for this program is determined by the State of New Mexico.
Step Two:
Once the enrollment application is approved by the MCO, you must make your first
premium payment before coverage can begin. Enrollment is dependent on health plan
approval and premium payment.
Once your payment is made, you will receive an identification card, information
about required premium payments, and a summary of benefits from your selected MCO.
Children who do not qualify for the PAK program because of health issues will be
referred back to Insure New Mexico! for other coverage options that may be available,
including the New Mexico Medical Insurance Pool.
